Add "fake" query parameters for task-title and substask-title to item URLs.

Currently item URLs contain the item ID number. When folks share URLs, which they do a lot, one can't tell what the item is about until one clicks into it. Adding the task-title and substask-title (if it's a substask) to the URL would make the URLs human readable and therefore much more useful. But they need to be "fake", as in unused, so that the URL still works based on the ID if someone changes the title.
